Pre-Screening Survey

  1. Are you 18 years of age or older?

  2. Is your cancer advanced (stage 3 or higher)?

  3. Are you suffering from pain?

  4. Are you currently using cannabis to alleviate symptoms from cancer related pain?

    If you are currently using cannabis to alleviate symptoms from cancer related pain, can you abstain from use for 2 weeks?

  5. Would you be interested in taking a cannabis-based investigational drug to try to control your pain level?

  6. Do you have a cardiac disease (e.g. history of unstable ischemic heart disease, heart failure, severe and uncontrolled hypertension)?

  7. Do you have a history of schizophrenia or bipolar disorder or previous psychosis or epilepsy or previous intolerance to cannabinoids?

  8. Do you have a history of addiction or potential abuse substances?

  9. Do you have a caregiver or a close family member or friend that could accompany you to the visits and answer a short questionnaire about your well-being?
